This document constitutes an official public offer (offer) by Life Dental Clinic (LDC) for the provision of dental and related services and defines the terms and conditions for receiving such services by individuals.

1. General Provisions

1.1. In accordance with Articles 633, 641, and 642 of the Civil Code of Ukraine, this document constitutes a public offer.

1.2. Acceptance (acceptance) of this Offer shall be deemed to include:

• booking an appointment via the website https://lifedentalclinic.com;

• completion of any form on the Website;

• contacting the Clinic by phone, email, or in person;

• actual receipt of dental services at the Clinic.

1.3. Acceptance of the Offer signifies the Patient’s full and unconditional acceptance of all terms of this Offer without any exceptions.

2. Parties to the Offer

2.1. Service Provider — Life Dental Clinic (LDC), operating as a sole proprietorship (FOP), EDRPOU code: 3233515671, Director — Pavlo Fedorenko, registered address: Kyiv, 36A Bilорусska Street.

2.2. Patient — any legally capable individual who has accepted the terms of this Offer.

3. Subject of the Offer

3.1. The Service Provider undertakes to provide the Patient with dental and related medical services, and the Patient undertakes to accept and pay for such services in accordance with the terms of this Offer.

3.2. The list of services, their indicative prices, and descriptions are published on the Website or communicated to the Patient during booking and/or consultation.

3.3. Information published on the Website is for informational purposes only and does not constitute medical advice.

4. Procedure for Providing Services

4.1. Dental services are provided exclusively after an in-person examination of the Patient by a doctor.

4.2. The treatment plan, scope of services, and their cost are determined by the doctor on an individual basis, taking into account the Patient’s health condition.

4.3. The Patient confirms that they provide accurate information about their health condition and bear responsibility for its completeness.

4.4. The Clinic reserves the right to refuse to provide services in cases stipulated by the legislation of Ukraine or for medical reasons.

5. Cost of Services and Payment Procedure

5.1. The cost of dental services is determined in accordance with the Clinic’s current price list.

5.2. The final cost of services may vary depending on the complexity of treatment, the Patient’s health condition, and the need for additional procedures.

5.3. Payment shall be made in the form and manner agreed between the Clinic and the Patient, in accordance with the current legislation of Ukraine.

6. Rights and Obligations of the Parties

6.1. Rights and Obligations of the Service Provider

The Service Provider has the right to:

• independently determine treatment methods;

• engage medical staff to provide services;

• receive payment for services rendered;

• refuse to provide services in the event of the Patient’s breach of the terms of this Offer.

The Service Provider undertakes to:

• provide services in accordance with applicable medical standards;

• maintain medical confidentiality;

• ensure an appropriate level of quality and safety of services.

6.2. Rights and Obligations of the Patient

The Patient has the right to:

• receive full information about services and treatment;

• respect for honour, dignity, and confidentiality;

• refuse treatment in accordance with the legislation of Ukraine.

The Patient undertakes to:

• follow the doctor’s recommendations;

• pay for services rendered in a timely manner;

• inform the doctor of any circumstances that may affect treatment.

7. Liability of the Parties

7.1. The Parties shall bear liability in accordance with the current legislation of Ukraine.

7.2. The Clinic shall not be liable for consequences arising from:

• the Patient’s failure to follow the doctor’s recommendations;

• the Patient providing inaccurate information;

• individual characteristics of the Patient’s body.

8. Processing of Personal Data

8.1. Acceptance of this Offer signifies the Patient’s consent to the processing of their personal data in accordance with the Privacy Policy published on the Website.

8.2. Personal and medical data are processed in compliance with the requirements of the Law of Ukraine “On Personal Data Protection”.

9. Term of the Offer

9.1. This Offer enters into force from the moment of its publication on the Website and remains valid indefinitely until revoked by the Clinic.

9.2. The Clinic has the right to amend or revoke this Offer at any time without prior notice to Patients.

10. Dispute Resolution

10.1. All disputes shall be resolved through negotiations.

10.2. If no agreement is reached, the dispute shall be subject to consideration by a court in accordance with the current legislation of Ukraine.
